Description Senior Pharmacy Technician - Providence St. Mary Medical Center - Walla Walla, WA Under the general guidance & direction of the pharmacist & as permitted by the respective state board of pharmacy, the role of the Medication History Specialist is to conduct patient, family, or caregiver interviews to obtain a list of medications a patient was taking prior to admission. If needed, the Specialist may utilize other data sources to further investigate and complete an accurate medication list. The medication list compiled by the Specialist will be entered into the hospital's computerized medication reconciliation program and verified by a registered pharmacist. On occasion, may perform pharmacy technician job duties as assigned. Key roles include: 1) Maintaining organization and efficiency to handle multiple responsibilities and quickly shift priorities in an environment of constant interruptions. 2) Maintaining meticulous attention to detail in day-to-day activities. 3) Communicating with individuals at all levels of the organization from patients to medical professionals. 4) Performing job functions efficiently in cramped and crowded work conditions. 5) Working with minimal supervision in high pressure situations. The caregiver must be sufficiently fluent in the English language to satisfactorily perform the essential functions of the position. The degree of fluency required will vary depending upon the nature of the position. For direct patient care roles: Performs and maintains currency of essential competencies as required by specific area of hire and populations served. Performs medication history assessments for new admissions to the hospital, providing complete information regarding each medication identified (drug, dose, route, frequency, indication for PRN, and last dose taken-date/time). Coordinates medication history assessment & Collects and clarifies home medication list through patient or caregiver interview, information gathered from electronic sources, community pharmacies and primary care provider & other sources such as state registries. Verifies patient allergy information and ensure complete documentation (substance, allergy category, reaction description). Provides the clinical pharmacists with completed medication histories and promptly identifies issues that require clarification/intervention Enters the home medication list into the electronic health record (EHR), as well as updating the patient's preferred community pharmacy information in the EHR. In addition, documents in the EHR and communication of relevant information associated with home medications in the EHR and/or verbally to the clinical pharmacist. Communicates concisely and effectively with patients, families, clinical pharmacists, and other health care providers related to patient home medication histories. Participates in performance improvement activities. Participates in the training of Pharmacy Technicians, Externs, and Pharmacy Students regarding home medication interviews and documentation. if needed, In addition to these specialized responsibilities, the Medication History Specialist will also be able to function as a standard pharmacy technician: Assists the pharmacists in preparing and distributing drugs and maintaining drug inventory.
