Job Summary:

Provides evaluation, reassessment, and treatment of patients referred for home health occupational therapy and supervises the plan of care and the clinical work of the COTAs assisting in the care. Serves as the home health case manager on therapy only patients taking responsibility for leading the care team, managing the total care plan, and assuring adherence to regulatory and agency guidelines for those cases.

Job Functions:

Completes accurate evaluations, reassessments, and treatments of OT patients. Oversees COTA treatments, assuring adherence to plan of care and quality outcomes. Develops patient plan of care and adapts the plan according to assessment data and patient's needs. Schedules work independently and according to patient needs/agency policy. Documents care, progress, goals, contacts, and orders according to policy/regulations. Serves as a care team member, communicating effectively with patients, families, caregivers, physicians, and third party payors. Works autonomously w/out direct supervision in the field.

Uses Computerized technology for clinical care effectively after on the job training. Provides appropriate patient and caregivers teaching. Serves as the primary case manager on Therapy Only cases. At Discharge, assures coordination with any needed on going services and completion of discharge teaching.

Provides state board required monthly supervision for COTAs. Determines which patients qualify for PCAs: contacts MD for orders, establishes PCA POC

and contacts scheduler to start process.

Education: Graduate of AOTA accredited OT program

Licensure: Kentucky licensed, Illinois license eligible
